1993 P Washington quarter strange looking

Can anybody tell me something about this quarter and the damage or error that it is. I’ve seen other coin similar to this and was wondering if it is some sort of a error from the mint or damaged after. I’d appreciate any input because i’ve ran across quite a few of these and have always been confused as to what it is.https://coinauctionshelp.com/forum/down ... w&id=70329

This is just one very damaged quarter. really no telling what damaged it.

Nothing in the minting process would make something like this. Yet many tools in a garage could :)
