Does anyone know what type of minting error this is? It's perfectly symmetrical on both sides of the coin.

There is nothing in the minting process which would be able to produce something like this. Its post mint damage. It looks like at some time in the past this silver dime was encased inside a good luck token of some sort. Use a search engine to see pictures of various old "encased coin good luck tokens."
