When polishing the dies used to strike coins, sometimes the workers can get over zealous and details are lost. Or sometimes elements can be missing b/c of what is called being struck through grease. Grease fills the elements in the die and so when a strike is made the metal cannot move into the die's engraved areas. Neither conditions normally bring any premium. It happens too often.
