It's a bronze Columbus Exposition Medal from 1893, they are a bit rare, but this one was made into a gift or love token. Reverse has been ground down. Neat medal and evidently some banking history there.
Very similar to the Exposititon Medal but not as nicely struck and there are some differences. One in particular is the Area of South America on the map at the top. The tip is pointing to a different are of the clasped hands.
I think the OP's medal is a knockoff of the Exposition medal and was presented by Bank of America of Italy to the individual at the top.

Nice catch, Cliff!
North America is too far left and South America is too accurate. On the real one, South America in "shrunk" in the East-West directions and the Baja Peninsula is visible.

WOW! 4" is a large medal. Too bad the Bank didn't present it in GOLD! hugeeyes
Also I've learned from everyone here to dig a little deeper when I think I have something figured out.
If it hadn't been for Daniels ID of the design, I would have still be trying to figure out the Italian and what appears to be Roman numerals running just inside the devices below the left side of the bust.
At first I wasn't sure if some of the slight differences and lack of detail I was picking up on was damage/wear until I got to the image of the Americas on the globe at the top.
It was fun to have that "Ahaaaa" moment.

Would they put all the markings of a authentic one on a fake and can it still be collectible

I don't know if it would be considered a counterfeit/fake. The expression "Imitation is the sincerest form of flattery" comes to mind. Just a very similar design, possibly produced by the same company. To me the fact they put their name on the Bank medal under the bust indicates they were not trying to deceive anyone.
As far a collectability, people collect everything! The value will mostly depend on how many are interested in collecting medals. It might appeal to some one collecting large medals, brass(?), banking related items, paper weights, etc.

The most expensive American collectible coin sits in the Smithsonian and its a counterfeit coin.
The history of a coin can be far more appealing than the actual coin. But, it has to have a history and demand.
So, anything is possible, just not very likely.
