I need some help figuring out how these errors could have occurred. The missing "T" in "UNITED" is grease filled die i believe. The indent around the perimeter at the rim on the obverse and the "notches" on the tops of letters in "Fillipinas" seem like maybe another coin wasn't ejected and the edge was struck into this coin? Then we have extra metal underneath the "S" in "STATES" and top left of both 4's in the date. Looking at "AMERICA" you can see a ghost of the word just beneath the original.
This, to me, seems like an awful lot of errors for one coin, did one thing happen at the mint to cause all this or was it a series of unfortunate events that day in Denver?

This coin has a substance on the surface and looks messed with.

Did you try soaking in acetone to see if any thing will come off?

It looks like it is glued into the holder but this is not errors. Removing the coin is the only solution to be 100% positive and no attributer is going to look at the coin and give an assessment as long as it remains in this holder.
