What caused ?

What caused the middle coin damage. the damage looks flush with the coin

It's been soldered and the solder has been smashed. These were commonly used a jewelry pieces.

interestingly, we see these quite often,.....
when old coins are/were used to make buttons, earrings, pendants, bracelets,..... so on.

so :agree: D, some sort of solder on them
